Skilled Nursing
Capital Post-Acute utilizes an Interdisciplinary Team approach for patient-centered care. Treatment goals and care plans are individualized after a thorough evaluation by our staff to ensure each patient’s specialized needs are met.

Short-Term Rehabilitation
Our rehabilitation program is designed to provide the highest level of rehabilitation and skilled nursing care to our patients in order to transition, as quickly and safely as possible, from hospital to home. Our state-of-the-art rehabilitation equipment and therapeutic modalities are the same as those used by Professional, Collegiate, and Olympic athletes to recover from injuries or surgeries.

Sub-Acute Care
Helping You Recover
Our sub-acute care offers a comfortable and healing environment while you recover from surgery, illness, or injury. We take patients, recently released from the hospital, and provide the around-the-clock care that is needed to get them back to their highest level of function so they can return home or to a lower level of care.
Dietary Program
Eating for Health
Our on-staff Registered Dietitian monitors weight deviations and makes recommendations accordingly. We provide three nutritious meals to meet specific dietary and therapeutic needs, according to physician orders such as regular, diabetic, cardiac, textured-modified diets, and regular nutritional assessments.
Social Services
Managing all Requests
The Director of Social Services manages all requests for missing items and concerns/grievances for our facility. Social services also assess our long-term population quarterly by maintaining all ancillary services for our residents ensuring that all basic needs are assessed routinely and as needed to promote the highest quality of life while in our facility.
